I got a big surprise when I filled my trimix prescription today. They gave
it to me in two vials...one already frozen, and had me do a consultation
with a pharmacist....first time in months they made me do a consultation.
The pharmacist said to keep the one vial frozen, and discard any unused
trimix or "triple P" in the first vial after fourteen days. The pharmacist
stressed the fourteen day spoilage was an absolute. I said I had always
stretched it for a full month. Then I am to thaw out the second vial, and
use it for no more than fourteen days.

I thought I had read here before that trimix should not be frozen, and that
some people stretch a vial out for three months or so before refilling.

The biggest shock was that the price had doubled....$143 for 4.5 ml. For
the last six months I have been paying $73 for it. I still only got 4.5 ml,
just it was two vials with 2.25 ml in each one. I was told there is a
phentolamine shortage right now, and that the price of phentolamine had
increased fivefold, and they are just passing on the cost of materials.

Anyone out there had an experience similar to this?

I get mine frozen, thaw it and fill a couple of syringse, refreeze the
vial and repeat the process over 3-4 months depending on activity with
no loss of effectiveness.

I'm nearly due for a refil and will report back if there's an
increase.

Haven't bought since Jan 5th and the price was
the same as it's been for just over 2 years.

By all means, stick with your pharmacist's
advice. Never hurts to ask for an explanation
though. If you get one, please post here.

I've been on 4ml frozen (labeled, "keep frozen")
since I started Trimix over two years ago. This
is Mayo (18/0.7/6)... about the weakest. I pay
$70 at a local compounding pharmacy. Mine is
labeled for 90 days but I've used it beyond 9
months.

I thaw, draw up two packs of syringes (20) and
refreeze both the vial and the syringes (needle
up). I pull several syringes from the freezer
and keep them chilled at 38-40 deg. Pharmacist
suggests either use or toss those within a month.

Twenty syringes is about 1/2 of a vial so I'm
thawing and refreezing two or three times over
the life of the vial. Vial is left unfrozen for
30 minutes at most. Pharmacist is OK with that.

A large school of medicine in Texas did a study
on the keeping properties of Trimix. I don't
recall the particulars but the paper that
discusses it is posted in the Yahoo ASI <files>
section. I'll hunt for a link and, if I find it
will post here.

Some pharmacists say to never freeze Trimix.
I've no clue if this is because of the
particular dilutant used in the mix or the
strength of the formula. This could also vary
from state to state since the various Boards of
Pharmacy have some differences in the
regulations for compounders.

"14 days" came from.....

USP 797 has provided some guidelines to
compounding pharmacists for setting what they
call the, "Beyond Use Date" (BUD).

The process isn't difficult but it has a lot of
factors that only the compounding pharmacist
will know. Just two of the factors are whether
the raw ingredients were sterile or non-sterile
and what means of sterilization was used during
teh actual compounding.

Some state boards require that compounders
comply with 797, other states have their own
rules. So, depending on where you buy Trimix,
you may find different BUDs for the same product.

This is a real YMMV situation and only the
Compounding Pharmacist has the answer.

Whether the vial is individually prepared or is
part of a larger batch (to be divided up into
separate vials) will also affect the BUD.

Table 4 in the second of the above references
suggests 14 days thawed and 45 days frozen for a
"low risk" item. Items classified as "high
risk" have much shorter BUDs.

Remember too that how you handle the vial is
important. My pharmacist cautions against
drawing up syringes in the bathroom or kitchen
since that injects 'dirtier' air into the vial
than if you had a 'clean room.'

Over the years, I have seen a tendency in pharmacies to gradually increase
the price every few months, but never a doubling from one month to the next.
I even quit Costco because they did this to me, from $85 to $135 on my 10 ml
TCyp, while the price stayed $85 on their website.

You are paying too much. I picked up mine today, and the price was the same
as usual, $49 for 5 ml of trimix. I asked the pharmacist if he had heard
anything about a phentolamine shortage or increase in price, and he said no.
He also said he had just received a shipment of phentolamine last week, and
the price was the same as he has paid for over a year.

Find a new pharmacy, fast!

So you are paying less than half what I was paying before the price doubled.
I was paying $77 for 4.5 ml. I need to find out if there is someplace I
can get a decent price.

My pharmacy doesn't ship but here's one that
does. University used to have a reputation for
being high priced but that was three years ago

I have contacted several out of area pharmacies, and now have an
understanding on why my pharmacy suddenly wants me to toss it out after 14
days. Federal guidelines now state that if the compounding lab is not a
sterile environment, the compound expires in fourteen days. That means that
in addition to doubling the price, it is not a sterile compound when I
receive it. I am still trying to get my doctor to authorize a prescription
to a mail order pharmacy.

I have found an online pharmacy who will supply me a 10 ml vial of a
stronger mix, for only $80.....only trouble is, I am having a time
convincing my doctor to write a prescription for their most common mix and
fax to them.

prescription. It got written and this is something that might be of
interest to others:

Hey, there is a Compounding pharmacy located in Texas that makes Tri-mix and gives a 4 month Beyond Use Date. The prices are $66 for 10ml and $38.50 for 5 ml. They are only licensed in about 20 states so makes sure they can ship to your state first. Items must be sent overnight which is another $32. Their name is Talon Compounding Pharmacy at 1-800 250-6232. Dr. can fax a prescription to 210-424-0026. You must have an Rx.
